How to find it
Go to the Spring of Courage in the Lake region. The shrine is at the spring, behind the prayer statue standing in the water. You need a Farosh‘s Scale, which Farosh drops when you shoot it.
Solution
Offer the scale
Drop a Farosh‘s Scale into the water at the statue’s feet. The door to the shrine opens.
Take the rewards
There are no puzzles. Climb the stairs, open the chest for the Thunderspear and speak to the monk for the Spirit Orb.
